WAKEFIELD — Genesis at Wakefield Center is focused on training all clinical staff to prevent and recognize diabetes complications early and to treat them appropriately. Collaborating with physicians, nurse practitioners and physician assistants will assure standards of diabetes medical care are met. Wakefield Center has an on-site Diabetes Care coordinator to monitor continuous quality care through management, evaluation and teaching. The center performs data collection to evaluate the impact of the program. It measures of success include increased staff competence in the care of diabetes patients, decreased hypoglycemic episodes, decreased hyperglycemic episodes, prevention of unnecessary emergency room visits and re-hospitalizations, the ability of patients with diabetes to choose a state-of-the-art facility in their own community, increased patient ability in diabetes self-care and management after discharge into the community from the facility.
